Job Description

This role involves ensuring appropriate reimbursement for patient care services and participating in the Patient Assessment Process. As a Minimum Data Set (MDS) Coordinator at Genesis, you will be responsible for developing individualized patient care plans, identifying patient needs, and coordinating assessments with an interdisciplinary team.


Responsibilities:
  • Develop and implement patient care plans that meet federal and state regulations
  • Participate in the Patient Assessment Process through completion of items on the MDS and accompanying Care Area Assessments (CAAs)
  • Assist with Activities of Daily Living training for new hires and ongoing continuing education for existing staff
  • Maintain knowledge of current RAI/Prospective Payment System regulations through educational programs

Required Skills and Qualifications:
  • Must be a Registered Nurse currently licensed by the state and in good standing
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility required
  • Prior experience completing MDSs and Care Plans required
  • Knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures, as well as laws, regulations, and guidelines governing long-term care
