Primary Care Medical Director

The Theoria Medical organization is seeking a highly skilled Primary Care Medical Director to lead our skilled nursing facilities (SNF) team.
About the RoleAs a key member of our leadership team, you will serve as medical director for multiple SNFs, participating in administrative and quality initiatives. Your expertise will be instrumental in guiding facility staff and supervising clinical staff to ensure high-quality patient care.
You will also collaborate with nurse practitioners (NPs) and physician assistants (PAs) to manage day-to-day resident care, providing guidance and support as needed. Your primary responsibility will be to provide high-quality, patient-centered primary care services by rounding on patients residing in SNFs.
Responsibilities- Attend quality assurance and process improvement meetings to drive excellence in care delivery
- Provide guidance and oversight to facility staff to ensure adherence to best practices
- Supervise facility clinical staff to ensure seamless patient care operations
- Rounding on patients residing in SNFs to deliver high-quality primary care services
- Diagnosing and treating patients with acute and chronic diseases using our proprietary EHR system
- Participating in Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives
We are seeking a qualified professional with:
- A strong background in primary care medicine, preferably with experience in SNF settings
- Excellent leadership and communication skills to effectively guide teams and facilitate collaboration
- A patient-centered approach to care delivery, prioritizing individualized attention and comprehensive support
- Ability to work independently and as part of a team, adapting to changing priorities and deadlines
- A commitment to quality and safety, adhering to regulatory standards and best practices
We provide a competitive compensation package, including a Medical Director stipend, and a comprehensive benefits program. Our flexible scheduling model allows for 2-3 days/week at each facility, with no nights, weekends, or on-call responsibilities. You will have the opportunity to participate in our Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives.
Our estimated annual salary range is $150,000 - $250,000, commensurate with experience and qualifications. Employer-paid life insurance policy and a dynamic work environment make this an attractive opportunity for the right candidate.
