Hospice Nurse

We are seeking a skilled and compassionate Hospice Registered Nurse / RN Case Manager to join our team. As a key member of our care team, you will be responsible for providing high-quality, patient-centered care to individuals in need of hospice services.

• Provide comprehensive care to patients, including assessing their physical, emotional, and spiritual needs.
• Develop and implement individualized care plans that address the unique needs and goals of each patient.
• Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
• Provide education and support to patients and their families, empowering them to make informed decisions about their care.
• Document patient progress and outcomes, ensuring accurate and timely reporting.

• RN license in the state of employment.
• Two years of experience as a registered nurse, with hospice experience preferred.
• Current driver's license.
• Current CPR and BCLS certification.
