As a Home Health Aide with CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care in the comfort of patients' homes. Your dedication and compassion will make a significant difference in the lives of our patients and their families.

Key Responsibilities:
  • Provide direct patient care under the supervision of a registered nurse and according to the patient's Plan of Care.
  • Assist patients with self-administered medications, ensuring their safety and adherence to prescribed treatments.
  • Take accurate temperature, pulse, and blood pressure measurements, recognizing and reporting any abnormal results to the supervisor.
  • Support patients with personal hygiene, including bathing, grooming, skin care, and other essential activities.
  • Assist with feeding patients, promoting healthy eating habits and observing food and fluid intake when necessary.
  • Help patients with mobility, exercises, positioning, transfers, and ambulation, as per the Plan of Care and CenterWell Home Health policy.
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care and accurate record-keeping.
  • Practice infection control principles, maintaining a clean, safe, and comfortable environment for patients.
  • Perform other household duties, such as light laundry, bed changing, and light meal preparation, as needed.
Requirements:
  • High school diploma or equivalent
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months
  • Meet applicable state certification requirements
  • Valid driver's license, auto insurance, and reliable transportation
  • Good standing on the HHA Registry (if applicable) and completion of HHA/CNA course
  • At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency
