Case Management Specialist

Jameco Home Health Agency Inc. is hiring a dedicated Case Manager to oversee patient care coordination.
This position comes with an estimated annual salary of $62,000, based on experience and qualifications.

Key Duties- Coordinate patient care by staffing and scheduling visits for skilled nurses, social workers, and home health aides;
- Monitor patient progress through regular communication and documentation;
- Oversee the assignment of supervisory visits for LVNs and CHHAs, ensuring timely submission of OASIS/visit notes;
- Manage durable medical equipment (DME) orders and supplies, as well as developing plans of care for CHHAs if applicable;
- Inform visiting staff of patient transfers or readmissions, maintaining open lines of communication;
- Meet the requirements outlined below:
- At least one year of experience in home health and/or case management;
- Able to pass a background check;
- Eligibility to work in the United States;
- Basic proficiency in Microsoft Office Excel, Word, and antivirus software;
- Strong attention to detail and note-taking skills;
- Excellent organizational skills;
- A positive attitude and team-oriented mindset;
- Willingness to learn and develop new skills;
- Availability for on-call duties using a company-provided cell phone, rotated among staff members.
