Job SummaryWe are seeking a skilled Registered Nurse to join our team as a Weekend Home Health PRN. As a Registered Nurse with Elara Caring, you will have the opportunity to work with a diverse range of patients and families, providing personalized care and support to help them achieve their health goals.
Key Responsibilities- Assess patient needs and develop individualized care plans to meet their physical, emotional, and social needs.
- Provide high-quality nursing care to patients in their homes, including wound care, medication management, and education on disease management.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
- Document patient progress and outcomes in a timely and accurate manner.
- Current, unrestricted state RN license.
- 1+ year experience in a clinical care setting as a nurse.
- Able and willing to travel within branch/office coverage area.
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
- Must be able and willing to travel 50%.
