Role Overview

POSITION SUMMARY:

The Senior Care Medical Practitioner (PCP) plays a pivotal role in our collaborative care model. As a value-driven provider, we prioritize quality care for our patients. Our care team comprises physicians, advanced practice professionals, pharmacists, care associates, medical assistants, behavioral health support, quality coders, and referral coordinators, all working together to create an unparalleled experience for seniors. This integrated approach enables our practitioners to focus on fewer patients, allowing for more dedicated time and attention to each individual.

Key Responsibilities

PRIMARY DUTIES:
  • Conduct evaluations and provide treatment for patients in line with established care standards.
  • Coordinate patient services, including specialty referrals, hospital coordination, and home health care.
  • Engage actively with the care team during daily meetings to share medical expertise.
  • Support the Lead Physician and Center Administrator in fostering a collaborative practice environment.
  • Ensure timely and accurate medical record documentation, collaborating with quality coders.
  • Adhere to policies and protocols set by Clinical Leadership.
  • Participate in discussions regarding quality of care and review of patient outcomes.
  • Focus on direct patient care while managing administrative responsibilities related to patient care.
Qualifications

REQUIRED QUALIFICATIONS:
  • Graduation from an accredited MD or DO program.
  • Possession of a current and unrestricted medical license or willingness to obtain one in the practice state.
  • Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
  • Active and unrestricted DEA license.
  • Medicare and Medicaid Provider Numbers.
  • Strong interpersonal and communication skills with colleagues and patients.
  • Commitment to a team-based care model.
  • Willingness to adapt to a value-based care environment.
  • Exceptional patient service skills.
  • Basic computer proficiency, including email and electronic medical records (EMR).
  • This position involves direct patient interaction and requires TB screening for selected candidates.
PREFERRED QUALIFICATIONS:
  • Familiarity with HEDIS/STARS metrics.
  • Understanding of CAHPS and HOS.
  • Experience in underserved or culturally diverse communities.
  • Proficiency with electronic health records.
  • Experience in Medicare Advantage Global Risk Models.
