Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team as a Home Health Care Specialist. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and well-being.

Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs, developing personalized care plans to address their unique requirements.
  • Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care techniques.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Coordinate with interdisciplinary teams, including physical therapists, occupational therapists, and home health aides, to ensure comprehensive care.
  • Document patient progress, including assessments, care plans, and outcomes, to ensure accurate and timely communication with healthcare providers.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum of one year of nursing experience, preferably in home health or a related field.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills, with the ability to work independently and as part of a team.
