Intensive Care Coordination Specialist

Position Overview: Carastar Health is looking for a committed professional to fill the role of Intensive Care Coordinator, focusing on providing support to individuals under 21 who are experiencing Serious Mental Illness (SMI) or Serious Emotional Disturbance (SED). In this vital position, you will be instrumental in linking these young individuals with crucial medical, social, and educational resources, thereby enhancing their quality of life.

  • Collaborate effectively with youth, families, and various child-serving systems
  • Implement the Wraparound Informed Approach to deliver tailored care
  • Coordinate thorough assessments and formulate individualized care plans
  • Provide crisis intervention and stabilization assistance
  • Help in building personal support networks and connecting with relevant resources
  • Engage in continuous evaluation and monitoring of individual progress

Key Responsibilities:

  • Conduct comprehensive needs assessments, care planning, and service coordination
  • Facilitate the establishment of Child & Family Care Teams
  • Develop individualized HICC Plans of Care and Crisis Stabilization & Support Plans
  • Offer crisis intervention, referrals, and social support services
  • Transport clients to necessary services and perform outreach visits
  • Participate in professional development activities to enhance competencies
  • Complete required reports and maintain accurate documentation

Essential Requirements:

  • Strong computer skills, including proficiency in Windows, typing, and Excel
  • Familiarity with Electronic Health Records and prompt documentation practices
  • Understanding of the unique needs and behaviors of SED children and adolescents
  • Skills in interpersonal communication, group dynamics, and interviewing techniques
  • Knowledge of psychotropic medications, confidentiality, and casework principles
  • Ability to document activities, maintain records, and provide services in diverse settings

Qualifications:

  • Bachelor's degree in behavioral science or a related discipline
  • At least one year of relevant experience
  • Approved Medicaid Provider status
  • Valid driver's license with a satisfactory driving record
  • Completion of an approved Intensive Care Coordination Curriculum
