This role offers comprehensive training, flexibility in schedules, and real work-life balance. As part of Allina Home Health, the leader in Minnesota for in-home care delivery, you will provide exceptional nursing care, allowing patients to recover where they are most comfortable.
Key Position Details- Every 4th weekend rotation
- Occasional on-call responsibilities
- Option of 8- or 10-hour day shifts

Nursing at Allina Health involves diagnosing and treating human responses to actual or potential health problems. This includes establishing an intentional therapeutic relationship between a registered nurse and a patient and family. As a leader and integrator of care, the professional nurse has the responsibility, authority, and accountability for planning, coordinating, and evaluating the patient's care needs.
The successful candidate will provide nursing care to patients and families requiring rehabilitation, medical intervention, advanced illness, or end-of-life care. Key responsibilities include:
- Comprehensive assessment of patient and family needs; identification of outcomes based on needs assessment; establishment of the plan of care; supervision of progress; and modification of the plan of care as needed.
- Maintenance of education and competencies to remain proficient in providing high-quality patient care practice and skills.
- Incorporation of evidence-based nursing practice, taking into consideration current statutes, rules, and regulations when developing the plan of care.
- Establishment, in collaboration with the family and patient, of realistic and measurable patient expected outcomes based on nursing diagnoses, patient present and potential capabilities, goals, available resources, and plan for continuity of care.
- Identification of outcomes that are age-appropriate, culturally appropriate, realistic, and attainable.
- Coordination, collaboration, and delegation of tasks to ensure care is provided in a safe and effective manner.
- Fulfillment of all organizational requirements.
- Supplemental roles may include Charge Nurse or Preceptor, demonstrating ability to coordinate, delegate, and direct unit operation, collaborate effectively with unit staff, leadership, and other disciplines.
- Other duties as assigned.
- Associate's or Vocational degree in Nursing
- 0 to 2 years RN experience
- Bachelor's degree in Nursing
- May consider new graduate RN
- 0 to 2 years homecare/hospice experience
- Licensed Registered Nurse - MN Board of Nursing required if working in the state of MN upon hire
- BLS Tier 1 - Basic Life Support - Multisource required by completion of orientation
- Valid Driver's License required upon hire - MN and WI residents must obtain a valid driver's license in their state of residence within 60 days of employment
- CHPN Certified Hospice and Palliative Nursing preferred
- RN-BC RN Case Management preferred
- ANCC-RNCM Certification in Case Management preferred
$68,000 - $80,000 per year, depending on experience and qualifications.
