Job DescriptionWe are seeking a compassionate and motivated Physical Therapist to join our team. As a Physical Therapist at JAG Physical Therapy, you will be responsible for assessing patients' strengths and weaknesses, creating effective treatment plans, and implementing interventions aligned with best practices.
You will work closely with interdisciplinary teams to provide patient care that is safe, effective, and in compliance with our standards. Your expertise will be valued and utilized to aid patients in achieving their goals and maximizing functional independence.
The ideal candidate will have a Bachelor's or Master's degree in Physical Therapy, a valid licensure in the state of practice, and experience working with EMR software. Strong clinical decision-making skills, excellent oral and written communication, and a positive professional demeanor are essential qualities for this role.
- Collects and records patient care data, including initial evaluations, daily notes, reassessments, progress reports, discharge plans, and communication with the team.
- Prepares for evaluations, obtaining patient history, researching unfamiliar diagnoses, and selecting and administering tests and measures.
- Formulates diagnoses, goals, prognosis, and plans of care, collaborating with interdisciplinary teams, patients, and support systems.
In addition to your clinical responsibilities, you will also oversee support staff, attend annual billing and coding seminars, promote JAG in the community through lectures and presentations, and maintain accurate records using Prompt EMR.
