**Job Summary**

We are seeking a skilled and compassionate Clinical Coordinator to join our team at Concord Care of Cortland. As a Clinical Coordinator, you will play a vital role in ensuring the delivery of high-quality patient care and promoting a positive work environment.

**Key Responsibilities**

• Develop and implement care plans that meet the unique needs of our residents
• Collaborate with the healthcare team to ensure seamless care transitions and effective communication
• Conduct regular assessments and evaluations to identify areas for improvement
• Provide education and training to staff on best practices and new policies
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

**Requirements**

• Current Registered Nurse License in the State of Ohio
• 2+ years of experience in Medicare/Medicaid areas in long-term care or clinical training
• Excellent communication and teaching skills
• Ability to work independently and efficiently
• Working knowledge of MDS 3.0, care plans, care guides, and CMI reimbursement process
