Position Summary:

Rothman Orthopaedics is seeking a dedicated Patient Navigator (Registered Nurse) to join our dynamic team. This role primarily allows for remote work with potential travel within the PA/SNJ region.

Key Responsibilities:
  • Patient Coordination: Collaborate closely with surgeons and allied health professionals across various specialties to ensure seamless communication and coordination of patient care pathways. Maintain and implement up-to-date coordinated care strategies while engaging with all healthcare team members on behalf of the patient.
  • Care Episode Management: Oversee the surgical patient’s journey throughout a 120-day care episode, encompassing 30 days pre-operation to 90 days post-operation.
  • Risk Evaluation: Assess patient risk factors related to surgical procedures and adhere to the appropriate care pathways.
  • Protocol Development: Partner with physicians to establish and refine clinical protocols.
  • Liaison Duties: Effectively communicate with referring physician offices as necessary to meet patient and physician needs.
  • Post-Discharge Coordination: Collaborate with outpatient physical therapy, home health agencies, skilled nursing facilities, and rehabilitation hospitals to ensure adherence to the coordinated care plan.
Qualifications:

Education: Associate's degree required; Bachelor's degree preferred.

Licensure: Active RN Licensure required; prior experience in Patient Navigation is preferred.

Experience: Minimum of 5 years as a Registered Nurse (RN).
