MDS Nursing Coordinator Opportunity

We are seeking a skilled MDS Coordinator to join our team at Canton Center for Nursing and Healing. As a valued member of our staff, you will play a critical role in ensuring our residents receive the highest level of care in a supportive and fulfilling work environment.

Key Responsibilities:
  • Review and process MDS assessments in accordance with CMS and LTCI regulations and guidelines.
  • Coordinate the development and revision of resident Interdisciplinary Care Plans based on RAI/MDS information.
  • Collaborate with nursing caregivers and other disciplines to ensure accurate care plan goals.
  • Prepare accurate reports on Quality Measure/Quality Indicators (QM/QI) from RAI/MDS data.
  • Active involvement in Quality Improvement initiatives.
