Job Summary

We are seeking an experienced RN Case Manager to join our team as a Clinical Nurse Coordinator - Case Management Services. In this role, you will work closely with interdisciplinary teams to assess, plan, and evaluate patient care, ensuring optimal outcomes through efficient resource utilization and discharge planning.

Key Responsibilities:

  • In collaboration with the care team, initiates care transitions and develops individualized discharge plans to meet patient needs and manage length of stay.
  • Evaluates at-risk populations and refers them to next level care to prevent readmission.
  • Serves as a case management resource expert, practicing autonomously to ensure compliance with regulatory requirements.
  • Works closely with the utilization department and physicians to ensure cost-effective care and best patient outcomes.
  • Identifies and resolves or escalates delays and obstacles to discharge, maintaining effective communication with the care team.
  • Evaluates daily patient plans and involves patients and families in formulating goals.
  • Maintains professional accountability through supporting patient rights, informed consents, and advanced directives.

Requirements and Qualifications

To be successful in this role, you must have:

  • A valid RN license
  • Strong clinical skills and experience in case management
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and collaboratively within a team

Salary and Benefits

The estimated salary for this role is $85,000 - $110,000 per year, depending on experience. Additionally, you will receive a comprehensive benefits package, including health insurance, retirement savings, and paid time off.
