Key Responsibilities
  • Collects, validates, synthesizes, and records objective and subjective data to inform patient care decisions.
  • Develops and implements individualized care plans to achieve patient goals and minimize hospitalizations.
  • Coordinates and delegates patient care tasks to ensure effective and efficient delivery of services.
  • Communicates and documents interventions and patient responses to ensure continuity of care.
  • Participates in quality improvement activities to enhance nursing practice and patient outcomes.
Requirements
  • Graduate of an accredited RN school of nursing.
  • Valid RN license in the state employed.
  • Valid CPR certification.
  • Ability to read, write, speak, and understand the spoken English language.
Preferred Qualifications
  • Prior skilled nursing experience.
  • Knowledge of policies and procedures and state and federal regulations.
Physical Demands

This role requires the ability to stand, walk, sit, talk, and hear for extended periods. The employee must be able to lift, carry, push, or pull objects weighing up to 50 pounds. The job requires close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
