Job Summary

Responsibilities
  • Patient Care & Education
    • Provide comprehensive patient care, including health education and disease management, to patients of all ages.
    • Conduct patient assessments, develop care plans, and implement interventions to promote optimal health outcomes.
    • Collaborate with healthcare providers to identify patient priorities and develop strategies to improve chronic disease management.
    • Provide medication education and counseling to patients and their families.
    • Perform basic emergency care and respond to medical emergencies as needed.
  • Documentation & Work Environment
    • Accurately document patient care and interactions in the electronic medical record.
    • Ensure a clean, safe, and functional work environment that promotes patient satisfaction and staff well-being.
    • Participate in quality improvement initiatives to enhance patient care and staff performance.
  • Case Management
    • Assess patient needs and develop care plans to address social determinants of health.
    • Collaborate with community resources to connect patients with necessary services and support.
    • Provide indirect case management services, including population health management and missed follow-up care.
  • Training & Development
    • Participate in ongoing education and training to enhance clinical skills and knowledge.
    • Assist in staff training and development programs to promote staff growth and development.
    • Mentor and orient new staff members to ensure a smooth transition into their roles.
Requirements
  • Equivalent to an LVN degree from an accredited school of licensed vocational nursing.
  • One to two years of increasingly responsible nursing experience, including experience in community nursing.
  • Possession of an appropriate, valid California Licensed Vocational Nursing License.
  • Possession of, or ability to obtain, an appropriate valid California driver's license.
  • Possession of, or ability to obtain, an appropriate valid BLS certificate.
  • Bilingual in English and Spanish, both written and verbal, with a language proficiency test score of Advanced or Intermediate.
