Physical Therapist PRN Job Description

We are seeking a skilled Physical Therapist to join our team on a PRN basis at Forestville Healthcare Center. As a member of our interdisciplinary team, you will have the opportunity to make a direct impact on the lives of our residents through therapy.

Qualifications:

  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
  • Valid or pending state physical therapy license

Job Responsibilities:

  1. Evaluate and review patient/resident medical charts, evaluations under physician order, and develop a treatment plan in accordance with patient needs.
  2. Implement treatment plan through direct treatment and education of the plan of care to all team members, including providing necessary supervision for PTAs.
  3. Attend and lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of the rehab team, as well as conducting staff/family training and education when appropriate.
