Job Summary

We are seeking a highly skilled Care Coordinator I or II to join our team at Sea Mar Community Health Centers. As a Care Coordinator, you will play a vital role in enhancing quality and patient-centered care by assessing gaps in care for patients with chronic conditions and/or mental health needs.

With over 90 medical, dental, and behavioral health clinics across Washington State, we offer a wide range of services including medical, dental, and behavioral health care, as well as nutritional, social, and educational programs.

Responsibilities

  • Participate in morning huddles to anticipate patient clinical, social and behavioral health needs.
  • Work with the care team to identify gaps in care and resolve them using process improvement strategies.
  • Provide brief interventions at point of care to assist patients with management of their chronic illness, address any social needs and link patients to behavioral health.
  • Advocate for patient services with community, social service, and medical providers.
  • Track patient's adherence with plan of care in electronic or paper charts and communicate outcomes and recommendations to the primary care provider.

Requirements

  • Bachelor's degree in health-related field or Medical Assistant Training with one or more years of experience in a community health setting or family practice.
  • Strong decision making and prioritization skills.
  • Excellent organizational skills and ability to handle a variety of tasks simultaneously.
  • Able to work respectfully and professionally with the community, patients, families and staff.

Salary and Benefits

The estimated annual salary for this position is $45,000 - $52,000 based on location and experience. We offer a comprehensive benefits package including medical, dental, vision, life insurance, long-term disability, EAP (Employee Assistance Program), paid-time-off starting at 24 days per year + 10 paid Holidays, and 401(k)/Retirement options.
