Our Mission


The Care Coordination Program is designed in alignment with the NYS DOH Health Home Model, which focuses on integrated systems of care to harmonize physical and behavioral health services, substance use treatment, and social support.

The primary objective of Care Coordination is to reduce hospital visits and emergency room usage by ensuring individuals have access to comprehensive services that promote their health and well-being.


Position Overview


The Client Enrollment Coordinator plays a crucial role in the initial outreach and registration of clients for our agency, leveraging provider partnerships and personal engagement.

The ideal candidate will demonstrate a strong ability to achieve a high enrollment success rate.

Key Responsibilities:


1. Conduct proactive outreach in the community and via phone to potential Health Home clients, along with necessary follow-ups.

2. Provide prospective clients and internal or external providers with a comprehensive overview of the New Horizon Counseling Center Health Home model.

3. Complete detailed client intake pre-screenings, assess Medicaid eligibility, and explain the Health Home process while meeting daily and weekly targets.

4. Assist clients with inactive Medicaid through the initial referral and entitlement process.

5. Maintain a conversion rate that meets or exceeds industry standards.

6. Engage in staff meetings and training sessions.

7. Consistently embody core values such as teamwork, empathy, flexibility, accountability, and empowerment.

8. Perform additional duties as assigned.


Qualifications


A High School Diploma is required.


Required Skills and Experience:


1. A minimum of one (1) year of experience in sales, marketing, or outreach, with a proven ability to connect with and engage the target client demographic.

2. A friendly, enthusiastic, and engaging personality, capable of effectively marketing and explaining the program to potential enrollees.

3. Sensitivity towards chronic conditions, substance use, mental health issues, and related challenges.

4. Proficiency in maintaining filing systems and electronic documentation.

5. Willingness and ability to conduct fieldwork and navigate public transportation.

6. Demonstrated sales/marketing experience with a track record of performing under pressure and achieving monthly targets.
