The Licensed Practical Nurse (LPN) Team Leader role is a dynamic position that combines direct patient care with the management of daily operations within our healthcare center, ensuring an efficient patient experience.
This role focuses on healthcare delivery and leadership, working closely with patients, their families, physicians, and other healthcare professionals in a vibrant and professional setting to provide exceptional quality care.
KEY RESPONSIBILITIES:
+ Serve in a dual capacity, assisting Primary Care Providers (PCPs) with patient care while overseeing daily center operations to ensure timely completion of tasks, with a focus on quality metrics.
+ Facilitate smooth operational workflows by addressing scheduling, patient wait times, and other critical areas to enhance service quality.
+ Collaborate with the Center Clinical Manager to supervise Care Promoters and support staff, contributing to performance evaluations and improvement plans as necessary.
+ Under the guidance of the Center Clinical Manager, train new and existing Care Promoters on health education, laboratory protocols, and medication management.
+ Prepare patients for examinations, conduct routine screenings, assist physicians during exams, and clearly communicate treatment plans and instructions to patients.
+ Manage patient inquiries via phone, ensuring effective communication and routing of messages to appropriate team members; coordinate prescription requests as authorized by physicians.
- In collaboration with the Center Clinical Manager, oversee quality assurance activities, including:
conducting equipment checks, monthly assessments, quality control for waived testing, and inventory management.
+ Engage in quality improvement initiatives that enhance patient safety and outcomes.
- Perform additional duties as assigned by management.
SKILLS AND QUALIFICATIONS:
- High school diploma or equivalent required.
- Graduation from an accredited practical or vocational nursing program required.
- Current, active LPN license in the state of employment required.
- IV Therapy Certification required if mandated by the state nursing board.
- Minimum of 1 year of experience as an LPN required; 3 years of overall clinical experience preferred.
- Experience with geriatric patients is advantageous.
- Familiarity with EMR systems preferred.
- Basic Life Support (BLS) certification from a recognized organization required within the first 90 days of employment.
