We are seeking a skilled Medical Social Worker to join our team at PruittHealth @ Home.
As a Medical Social Worker, you will provide medical social services to our patients and their families in their place of residence.
You will work closely with our interdisciplinary team to develop and implement plans of care that meet the unique needs of each patient.
The ideal candidate will have a Master's degree in social work, one year of experience in medical social work, and valid driver's license and automobile liability insurance.
Responsibilities:
Complete psychosocial assessments to identify patient needs and develop plans of care.Collaborate with interdisciplinary team members to ensure comprehensive care.Locate and obtain resources to support patient well-being.Maintain accurate and timely documentation of patient interactions and progress.Communicate effectively with patients, families, and healthcare providers.What We Offer:
