Job Summary:

The LPN/LVN Clinical Coordinator will function in collaborative practice with physician(s) and/or nurse practitioner(s) in a physician practice or clinic setting. The successful candidate will provide ambulatory nursing care as directed by physician(s) and/or nurse practitioner(s), including patient and family assessment, intervention, and education.

Responsibilities:
  • Provide high-quality ambulatory nursing care to patients and families in a physician practice or clinic setting.
  • Collaborate with physician(s) and/or nurse practitioner(s) to develop and implement patient care plans.
  • Conduct patient and family assessments, identify health needs, and develop interventions to address those needs.
  • Provide education to patients and families on health promotion, disease prevention, and management of chronic conditions.
  • Supervise licensed and unlicensed personnel, and coordinate the daily clinical operations of the multi-physician practice.
Requirements:
  • Graduation from an accredited school of vocational nursing.
  • Minimum 2 years of experience in a physician practice or clinic setting, with a preference for pediatric experience.
  • Current license to practice vocational nursing in the State of Texas.
  • Current CPR certification.
