We are seeking a highly skilled Patient Care Specialist to join our team at Brickyard Healthcare.

About the Role:

This is an exciting opportunity for a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with experience in long-term care settings to take on a challenging and rewarding role as our MDS Coordinator.

Responsibilities:
  • Resident Assessment Instrument Completion: Ensure compliance with federal and state regulations by completing the Resident Assessment Instrument according to company policy and procedures.
  • Case Management: Coordinate services with physicians, families, third-party payers, and facility staff to provide comprehensive care for residents.
  • A full job description outlining all responsibilities and physical requirements will be provided during the interview process.
Requirements:
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current, active license in the state of practice.
  • Current CPR certification.
  • Minimum two years of clinical experience in a healthcare setting.
  • At least one year of experience in a long-term care setting.
  • Prior experience as an MDS Coordinator with demonstrated expertise.
  • Able to maintain regular attendance.
Benefits and Compensation:

We offer a competitive salary range of $65,000 - $85,000 per annum, based on experience and qualifications.
