We are currently seeking a dedicated individual to oversee patient care as the Registered Nurse. In this pivotal role, you will be responsible for conducting care conferences, assisting with resident evaluations, and ensuring that residents receive appropriate levels of care. Your duties will also include monitoring medication administration and care plans, auditing medication carts, and maintaining resident files. This position requires a balance of administrative responsibilities and leadership authority to guarantee the highest quality of care for our residents.

Key Responsibilities:

  • Evaluate residents prior to their admission, when feasible.
  • Conduct visits and reassessments for current residents in acute care or other long-term care facilities.
  • Apply the nursing process through thorough assessment, planning, implementation, and evaluation of resident care, as observed by staff.
  • Oversee medication management by collaborating with pharmacy and team members to ensure accurate medication orders and availability.
  • Communicate effectively with resident Power of Attorneys and Primary Care Physicians regarding medication adjustments.
  • Monitor electronic documentation, including observations and incident reports, and follow up with relevant directors on any outstanding issues or serious clinical concerns.
  • Conduct monthly reviews of PRN psychotropic medications and quarterly assessments of routine psychotropic medications.
  • Engage in auditing processes as required and participate in regulatory compliance efforts.

Qualifications:

  • Bachelor's or associate degree in nursing is preferred.
  • Must possess a current, unencumbered active license to practice as an RN or LPN in the state of Wisconsin or be qualified to obtain such a license.
  • A minimum of one year of experience in Long-Term Care is preferred.
