**Job Overview**

We are seeking a skilled Nurse Practitioner to join our team for Home Health Risk Assessments in Chester, SC.

This is a part-time position with flexible scheduling and the opportunity to make a meaningful impact on the health and well-being of our patients.

**Compensation & Benefits**

The estimated annual salary for this role is $55,000 - $60,000 based on location and experience. Additionally, you will receive mileage reimbursement and opportunities for professional growth and development.

**About the Position**

As a Nurse Practitioner, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families. You will work collaboratively with our interdisciplinary team to ensure seamless care coordination and exceptional patient outcomes.

You will have the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth appointments. Our flexible scheduling allows you to choose shifts that fit your needs, with options for days, evenings, nights, and weekends.

**Responsibilities & Qualifications**
  • Conduct thorough health assessments, medical history reviews, and develop individualized care plans
  • Collaborate with healthcare teams, including physicians, social workers, and case managers
  • Provide patient education and support to promote healthy behaviors and disease management
  • Document all interactions, assessments, and care plans accurately and timely
  • Maintain up-to-date knowledge of relevant laws, regulations, and best practices

To be successful in this role, you must possess:

  • A Master's degree in Nursing or commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP licensure in the state of practice, with prescription authority or the ability to obtain it
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • 1+ year of experience as a Nurse Practitioner, with strong computer skills and familiarity with electronic health records
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
**Travel Requirements**

You must be able to travel frequently to meet patients in their homes or other settings. A valid driver's license and proof of adequate automobile insurance are required.
