Job Summary

The Nurse Practitioner will provide holistic, compassionate care to members and families in collaboration with physicians, staff RNs, and other healthcare professionals. This role involves diagnosing, managing, and preventing acute and chronic illness and disease, as well as promoting wellness.

Responsibilities
  • Performs in-depth health risk assessments on assigned members
  • Works collaboratively with physicians, case managers, social workers, family members, and other healthcare professionals
  • Identifies quality improvement activities and plans individualized healthcare consistent with policies and protocols
  • Reviews and interprets medical, surgical, and social history of members
  • Performs screening procedures and interprets laboratory and diagnostic data
  • Prescribes or performs various treatment modalities, including medications, injections, and wellness counseling
  • Refers members to PCP, case management, and disease management programs as necessary
  • Maintains confidentiality and adheres to HIPAA regulations
Requirements
  • Graduated from an accredited Adult/Gerontological Primary Care Nurse Practitioner program
  • Current RN and NP licensure in the state of practice
  • Board certification by AANP or ANCC in Adult, Family Practice, and Geriatrics or Adult-Gerontology Primary Care
  • Current CPR certification
  • Geriatric certification (obtained within 18 months of employment)
Preferred Qualifications
  • Bilingual (English & Spanish)
  • Knowledge of community resources
  • Previous experience working with behavioral health populations
  • Experience working with low-income and disadvantaged populations
