Care Coordinator Registered Nurse (RN)

West Side Family Health Care is seeking a skilled and compassionate Care Coordinator Registered Nurse (RN) to support our clinic operations. As a key member of our care team, you will work closely with our Care Coordinator Supervisor to implement patient care plans and provide essential services for patients' medical, behavioral, and social needs.

Key Responsibilities:

  • Support clinic operations, including quality programs, patient education, and data management.
  • Collaborate with the Care Coordinator Supervisor to develop and implement patient care plans.
  • Provide administrative, clerical, and clinical duties under the direction of a physician and within the scope of practice as governed by the Medical Board of California.

Requirements:

  • Possess an active license as a Registered Nurse and at least 2 years of experience in a healthcare setting, or
  • Have at least 2 years of experience as a Case Manager in a healthcare setting.

Preferred Qualifications:

  • BLS/CPR Certification
  • Bilingual Spanish/English-Healthcare Versed
  • Case Management experience
