1. Participate in initial and ongoing assessments of patients, identifying their unique needs and developing effective plans of care.
2. Collaborate with interdisciplinary teams to coordinate care, ensure seamless transitions, and promote positive patient outcomes.
3. Administer medications, treatments, and other interventions as prescribed by physicians and according to established guidelines.
4. Maintain accurate and detailed records of patient interactions, progress, and treatment outcomes, adhering to HIPAA standards and regulatory requirements.

To excel in this role, you must possess the following skills and qualifications:

• Certified Licensed Vocational Nurse (LVN) licensure in the State of California.
• Previous experience in a patient care setting, preferably in a hospital or clinic environment.
• Strong analytical and problem-solving skills, with the ability to think critically and make informed decisions.
• Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
