Key Responsibilities- Conduct thorough assessments of patients' psychosocial needs and develop individualized treatment plans.
- Provide evidence-based behavioral and medical health interventions, including psychotherapy and education.
- Collaborate with patients, families, and healthcare teams to develop comprehensive discharge plans.
- Assess and address social determinants of health and provide referrals as needed.
- Maintain accurate and timely documentation of patient care.
- Master's degree in Social Work (MSW) or related field.
- 0-3 years of experience in social work or related field.
- Ability to work under supervision and provide crisis intervention services.
- Excellent communication and interpersonal skills.
- Proficient in Microsoft Office and/or Google Suite platforms.
