Job Summary

The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality patient care at Wellstar Health System. Under the direction of a Registered Nurse or Manager, the LPN functions within the LPN Scope of Practice, delivering direct patient care to patients with medical/surgical needs.

Key Responsibilities
  • Direct Patient Care
    • Assess and assist with managing patients' total needs following physician orders with RN oversight and utilization of the nursing process.
    • Perform routine nursing procedures to meet the needs of the patient under the supervision of RN.
  • Coordination of Services for Episode of Care
    • Coordinate care and services with RN and other team members.
    • Inform RN, therapist, and/or clinical supervisor of changes in patients' condition, as appropriate.
    • Collaborate with RN to develop and revise individual plan of care based on patient need and physician order.
  • Patient/Caregiver Education
    • Serve as patient educator to foster self-care or caregiver support for the home patient.
  • Staff Education and Professional Development
    • Participate in the orientation and education of new employees and nursing students as appropriate.
    • Responsible for attending continuing education and in-services as directed and needed.
  • Communication and Documentation
    • Responsible for accurate completion of required documentation within established timeframe.
    • Demonstrate knowledge and appropriate application of regulatory guidelines.
    • Participate in care team conferences with co-workers.
Requirements
  • Graduate of an accredited School of Practical Nursing.
  • Current State of Georgia professional licensure.
  • Lic Practical Nurse.
  • Basic Life Support or BLS - Instructor.
  • Georgia Driver's License (preferred).
  • Private Vehicle insurance (preferred).
