As a Licensed Practical Nurse (LPN) on our Home Health team, you will play a vital role in providing high-quality care to patients in their homes. Your expertise and compassion will make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:

  • Provide direct nursing care to home health patients, following established plans of care and collaborating with the interdisciplinary team.
  • Conduct regular home visits to assess patient needs and provide care, including administering medications and performing approved nursing procedures.
  • Document patient interactions, interventions, and outcomes in the medical record, ensuring accurate and timely documentation.
  • Participate in performance improvement activities and contribute to the development of care plans that meet the unique needs of each patient.

Requirements:

  • Basic Cardiac Life Support (BLS) certification
  • Valid driver's license and reliable personal vehicle
  • Pennsylvania and/or New Jersey LPN licensure, depending on geographic location and scope of responsibilities
