Responsibilities
  • Implement and monitor patient care plans, ensuring the highest level of care and attention to detail.
  • Coordinate with the healthcare team to provide well-coordinated patient care, including serving as a primary point of contact for patients and families.
  • Assess and coordinate patient discharge planning needs, ensuring a smooth transition to home or further care.
Requirements
  • BLS Provider certification within 1 month of hire date required.
  • Licensed Associate Professional Registered Nurse credentialed from the Alabama Board of Nursing required.
