Key Responsibilities- Coordinate patient referrals to medical/surgical specialty physicians and/or outpatient services.
- Coordinate Neighborhood Help (NHELP) interdisciplinary team referrals.
- Coordinate and track clinical referrals for Care Transition Clinic (CTC).
- Lead the coordination of Jackson Health Center financial assistance referrals by working with providers, the outreach team, Jackson Health system, and students to aid patients in obtaining approval to close the loop on medical referrals.
- Provide patient/family education and instructions as delegated by the Provider and/or outpatient referral source.
- Document all instruction/education, and conversations with the patient into the medical record.
- Review lab and other diagnostic study results and communicate critical or abnormal values to the Physician.
- Provide patient/family education and home care instruction as delegated by the Physician and document all instruction/education provided in the medical record.
- Maintain patient and business confidentiality at all times and treat patients, the public and co-workers in a professional manner.
- Conduct daily medical record review activities to ensure that documentation is accurate, complete, timely, and appropriate.
- Intermittently assist on the Mobile Health Center with vital signs, intake, phlebotomy, immunization administration, medicines, treatments ordered by the physician. Document all care given in the electronic medical record.
- Participate in Quality Improvement. Assist in the evaluation of the quality of patient care and patient satisfaction, makes recommendations for improvement, and formulates treatment plan to meet patient needs.
- Perform other related duties as required or directed.
- High school diploma or equivalent and three (3) years of related nursing experience; OR an equivalent combination of relevant education and/or experience.
- Basic Cardiac Life Support.
- Licensed Practical Nurse.
- Graduate of accredited school of Practical Nursing.
- Experience preferably in a physician practice or clinic environment.
- Phlebotomy and Immunization Administration experience.
- Bilingual English/Spanish preferred.
- Experience working with Electronic Medical Records.
