Optum is seeking a skilled Nurse Practitioner to join our Home-Based Medical Care team in Mebane, NC. As a member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.
Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans for patients with complex medical needs
- Collaborate with patients, caregivers, and healthcare providers to ensure seamless care coordination and communication
- Prescribe appropriate diagnostics and interventions to prevent unnecessary hospitalizations and improve patient outcomes
- Provide education and support to patients and their families on disease management and wellness strategies
- Participate in ongoing quality improvement initiatives to enhance patient care and outcomes
- Current state RN license and NP license
- Completion of a Nurse Practitioner accredited program
- Knowledge of long-term care, community resources, and cost-effective alternatives for patient care delivery systems
- Access to reliable transportation
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of CPT, ICD-10, and HCPC codes
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
