We are seeking an Occupational Therapist to join our team at Autumn Lake Healthcare at Birch Manor. As a skilled nursing facility, we provide exceptional care and rehabilitation services to our patients.
This role involves assessing patient physical, communication and cognitive skills to develop personalized treatment plans. You will work closely with patients, their families, and healthcare teams to achieve optimal rehabilitation outcomes.
Key Responsibilities
- Develop and implement treatment plans to improve patient motor skills and daily functioning.
- Conduct thorough assessments of patient abilities and needs.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Maintain accurate records of patient progress and treatment plans.
Requirements and Qualifications
- Active state OT license required.
- Strong written and verbal communication skills.
- Ability to work effectively in a team environment.
- Documented experience in skilled nursing facilities or similar settings preferred.
