Job Summary

We are seeking a skilled and compassionate Home Health RN to join our team at Kaiser Permanente. As a leader of the health care team, you will provide professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures.

Key Responsibilities
  • Develop and implement individualized plans of care that reflect assessment, planning, implementing, and evaluating outcomes.
  • Ensure patient safety by utilizing the five rights, preventing falls, decubitus prevention, and nosocomial infections.
  • Provide optimal pain control and patient comfort, identifying and discussing patient anxieties, fears, or concerns.
  • Ensure patients understand medication purposes, side effects, and administration instructions.
  • Practice customer service standards, promptly answering call lights, alarms, and patient requests.
  • Maintain and protect patient confidentiality, ensuring a clean, orderly, and functional work environment.
Requirements
  • Minimum one (1) year recent experience in a licensed Home Health, Palliative Care, and/or Hospice agency as an RN/PHN.
  • Registered Nurse License (California).
  • Clean Driving Record for the last two (2) years.
  • Proof of automobile insurance as required by law.
