Job Title: Direct Support Professional

At Mt Olivet Rolling Acres, we are seeking a dedicated and compassionate Direct Support Professional to join our team. As a Direct Support Professional, you will play a vital role in supporting individuals with intellectual and developmental disabilities, as well as mental health needs, in their daily lives.

Job Summary:

The Direct Support Professional will assist clients with all aspects of daily living, including personal care, medical needs, and daily routines. This role requires a strong understanding of person-centered services and the ability to implement programs in accordance with the Coordinated Service and Support Plan (CSSP) and Coordinated Service and Support Plan Addendum (CSSP-A).

Key Responsibilities:
  • Assist clients with daily cares, medical needs, and supports, ADLs, and mental health supports.
  • Assist individuals with a daily routine, providing opportunities for choice and presenting creative options as needed.
  • Plan and implement recreation, leisure, and community activities.
  • Drive to/from and accompany clients at appointments and community activities using a company vehicle during all hours of operation.
  • Assist individuals supported in establishing and maintaining community relationships and facilitating friendships.
  • Demonstrate an understanding of the person as a unique individual and how to implement programs in accordance with the CSSP and CSSP-A.
  • Provide person-centered services demonstrated through practice, action, and language.
  • Maintain a working knowledge of person(s) history, skills, areas of vulnerabilities, habilitation plans, and emergency procedures.
  • Participate in the support team process; assist with developing, and implementing goals.
  • Maintain and perform proper administration and documentation of all prescribed medication, treatments, diets, and/or exams, or as otherwise directed by the CSSP and CSSP-A.
  • Maintain confidentiality in accordance with HIPPA and MORA policies and procedures.
  • Prepare meals following planned menu for clients in the home and complete weekly food ordering as needed and/or scheduled.
  • Assist with daily housekeeping and household maintenance.
  • Complete house quality improvement assessments and address identified issues as needed.
  • Ensure adequate supervision of clients, including remaining on shift until a replacement employee arrives.
  • Work harmoniously with and show respect to all internal and external individuals.
  • Communicate effectively with others at work verbally and in writing, using the primary spoken language of clients in the home.
  • Complete all mandatory training either determined by MORA or 245D regulations.
  • Read and follow company policies and procedures.
  • Remain awake at all times unless working an assigned sleeping shift.
  • Document program data and daily log information accurately and in a timely fashion.
  • Collaborate with the team to complete other duties as assigned or as needed.
Requirements:
  • At least 1 year of experience with clients with intellectual/developmental disabilities.
  • Interest in supporting clients with Intellectual and Developmental Disabilities, and/or mental health needs.
