Community Care Worker I

As a Community Care Worker I with the Health & Human Services Comm, you will play a vital role in determining the needs and eligibility of individuals for Community Care services. Your primary responsibilities will include conducting home visits, interviewing individuals, documenting information, and developing service plans. You will also verify data, explain program benefits and requirements, refer individuals for appropriate services, and ensure authorizations are registered in automated systems.
Key Responsibilities:
• Conduct home visits and interviews to assess the need for services and provide information and referral to other resources.
• Determine eligibility and functional needs through reviews and assessments.
• Obtain, verify, and calculate income and resources to determine financial eligibility for Title XX programs.
• Maintain case record documents and enter information in an automated system, establishing a record for each individual receiving service.
• Monitor services to determine if they are meeting the individual's needs and develop/coordinate/review service plans accordingly.
• Prepare basic level ongoing or special narrative or statistical reports as required.
Requirements:
• Ability to effectively communicate orally and in writing.
• Ability to set priorities, establish timeframes, and meet deadlines.
• Ability to establish and maintain effective relationships with individuals receiving services, co-workers, contract agency staff, and staff from other federal or state agencies.
• Ability to operate computer and general office equipment.
• Knowledge of interviewing techniques to obtain personal information, to make inquiries, and to resolve conflicting statements.
• Knowledge of resources that serve individuals who are older or have a disability.
Preferred Qualifications:
• Education and/or experience indicating the individual possesses the knowledge, skills, and abilities necessary for job performance.
• Experience using email and other automated applications, such as Microsoft Office products.
• Ability to travel approximately 70% of the time to make home visits, attend trainings, and/or meetings.
• Experience communicating effectively in writing.
Additional Information:
Interviews will be conducted using Microsoft TEAMS application. Selected applicants should download Microsoft TEAMS app prior to scheduled interview. This position is included in a career track series and may be filled at any level within the career track as a Worker I, Worker II, or Worker II I level, dependent on the selected applicant's qualifications as they relate to career track promotion eligibility requirements. Applicants with previous employment with CCSE/CCAD will be reviewed by program management to determine appropriate entry level. Occasional overtime and/or extended work hours may be needed for the position. Applicants may not have a history of substantiated fraudulent activity against HHSC or any programs it administers.
