Key Responsibilities- Collaborate with patients, families, physicians, and multidisciplinary teams to identify and address patient needs and develop comprehensive care plans.
- Coordinate patient care progression, ensuring timely and efficient transitions between healthcare settings.
- Assess and respond to patient/family needs, identifying and resolving barriers to effective care.
- Develop and implement discharge plans, coordinating with payors, agencies, and vendors as necessary.
- Act as patient advocate, negotiating for and coordinating resources to support patient care and well-being.
- Ensure continuity of care through effective communication and documentation.
- Provide education, information, and support to patients and families, promoting informed decision-making and self-advocacy.
- Collaborate with healthcare teams to promote appropriate use of medical center resources and optimize patient outcomes.
- Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required.
- Two or more years clinical experience, with one year in the acute care setting preferred.
- RN or LMSW in the state of employment required for new hires.
- Certification in Case Management preferred.
