St. John's Episcopal Hospital is seeking a skilled Social Worker to join their team. The ideal candidate will provide high-quality patient care, working closely with families and healthcare professionals.

Job Summary:

• Conduct thorough psychosocial evaluations to identify patients' needs and develop effective treatment plans.
• Collaborate with patients, family members, and care partners to create personalized services, ensuring seamless transitions and continuous support.
• Deliver individual and group psychotherapy sessions as outlined in treatment plans, promoting emotional well-being and empowerment.
• Participate actively in departmental meetings, sharing expertise and best practices to enhance overall care.
• Maintain accurate records according to clinical standards, documenting progress and interventions.
• Develop comprehensive aftercare plans for patients upon discharge, establishing connections to relevant community resources.
• Demonstrate proficiency in suicide risk assessment and telehealth services, adhering to federal and state regulatory guidelines.

Requirements:

• Master of Social Work degree from an accredited institution.
• Valid New York State Licensed Clinical Social Worker (LCSW) certification.
• At least 2 years of experience in Behavioral Health Outpatient settings, preferably bilingual in Spanish.

About the Opportunity

This Full-Time position offers a competitive salary range of $42.69 - $58.06 per hour, depending on experience, and a schedule that includes 8:30 AM - 5:00 PM shifts three days a week, with occasional late evenings.
