Job Summary:We are looking for a highly skilled and compassionate Occupational Therapist to provide patient care services, including direct treatment and follow-up, to our patients in their homes. The ideal candidate will have a strong knowledge of occupational therapy principles and practices, as well as excellent communication and organizational skills.
Responsibilities:- Evaluate patients new to service and develop a plan of care based on their diagnosis and assessment findings.
- Provide patient care services, including direct treatment and follow-up, to patients in their homes.
- Coordinate with other healthcare professionals, including COTAs and Patient Care Coordinators, to ensure seamless care delivery.
- Document patient care data accurately and thoroughly, reflecting evidence of improvement or decline in patient condition.
- Communicate effectively with patients, families, and healthcare professionals to ensure optimal care outcomes.
- Minimum bachelor's degree in occupational therapy from an accredited school of occupational therapy.
- Successful completion of the National Certification Exam and current licensure from the state where therapy will be provided.
- Prefer a minimum of one year's therapeutic experience in a non-home health care environment.
- Strong knowledge of occupational therapy principles and practices, as well as excellent communication and organizational skills.
- CPR certification and ability to work in a fast-paced environment.
