Job Description

As a Targeted Case Manager at AWARE, you will be responsible for providing case management services to individuals who meet the definition of being developmentally disabled. This includes assisting these persons gain access to medical services, social services, educational services, vocational services, and recreational services.

You will also be responsible for managing caseloads and/or billing requirements on a monthly, quarterly, and yearly basis. If you are passionate about making a difference in the lives of others, we encourage you to apply for this rewarding opportunity.

  • Provides case management services to individuals who meet the definition of being developmentally disabled, as defined by the State of Montana.
  • Assists these persons gain access to the following:
  • Medical Services
  • Social Services
  • Educational Services
  • Vocational Services
  • Recreational Services

Required Skills and Qualifications

To be successful in this role, you will need to possess a BS/BA degree in a social work related field and have a minimum of one (1) year of direct work experience in the developmental disability field. You must also have the ability to develop and implement individual treatment plans, with knowledge of behavioral, clinical, and mental health interventions, and skill building techniques.

  • Understanding of the emotional and behavioral needs of assigned clients.
  • Interpersonal communications skills necessary to; work independently and as a team member, make observations and to report them accurately, and follow oral and written instructions.
  • The ability to facilitate meetings.
  • The ability to work independently via a virtual office; using a lap top computer, and cell phone.
