We are seeking a dedicated and compassionate Home Health Aide to join our team at CenterWell Home Health. This is a per diem opportunity that allows you to work on a flexible schedule, providing personalized care to patients in the comfort of their own homes.
About the RoleAs a Home Health Aide with CenterWell Home Health, you will have the opportunity to make a meaningful difference in the lives of our patients. You will be responsible for providing direct patient care under the direction of a registered nurse and according to the patient's Plan of Care (POC). Your duties will include assisting patients with self-administered medications, taking accurate temperature, pulse, and blood pressure measurements, and recognizing and reporting abnormal results to your supervisor.
You will also assist patients with maintaining good personal hygiene, including bathing, grooming, skin care, shaving, oral care, nail/foot care, and other activities. Additionally, you will help patients with feeding, safely position them for meals, and feed or assist with self-feeding. You will also assist with patient toileting, including the use of bed pans/urinals, changing and positioning catheter bags, and performing bag change procedures on well-regulated ostomies.
Your role will also involve providing necessary skills to safely assist patients with mobility, exercises, positioning/turning, transfers, and ambulation, all while following the patient's Plan of Care and CenterWell Home Health policy. You will document interactions with patients, caregivers, doctors, and other staff members accurately, legibly, thoroughly, and within the allotted time.
RequirementsTo be successful in this role, you must have a high school diploma or equivalent, completion of a Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months, and meet applicable state certification requirements. You must also have a valid driver's license, auto insurance, and reliable transportation, as well as be in good standing on the HHA Registry (if applicable) and have completed an HHA/CNA course to work for a Medicare-certified agency.
A minimum of one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency is required. Compensation and BenefitsThe estimated annual salary for this role is $32,500-$44,700. We offer a range of benefits to support your whole-person well-being, including paid time off, a 401(k) retirement savings plan, employee assistance program, business travel, and accident coverage.
