Responsibilities:
Conduct in-home assessments and outpatient clinical work to identify diagnoses and develop care plans for patients with chronic disease management needs.
Communicate findings to inform PCPs of potential gaps in care and provide patient/family/caretaker education on close monitoring and follow-up of patient needs.
Emphasize knowledge of appropriate community resources for referral and comply with all HIPAA regulations and maintain security of protected health information (PHI).
Requirements:
Master's degree from an accredited NP Program or PA program.
Active California state Nurse Practitioner or Physician Assistant license.
Active RN License and Furnishing number (Must, upon hire) NP only.
Active Nurse Practitioner Board Certification/Physician Assistant Certification.
NPI Number, DEA, Valid BLS.
Valid California driver license and current automobile insurance.
