Job Summary

This Home Health Registered Nurse position is a 13-week contract with a reputable healthcare organization. The ideal candidate will have prior experience with OASIS/Medicare Certified Visits and be able to work independently with minimal supervision.

Responsibilities
  • Provide high-quality patient care in a home health setting
  • Conduct thorough assessments and develop individualized care plans
  • Collaborate with interdisciplinary teams to ensure seamless care transitions
  • Document patient progress and outcomes accurately and timely
  • Participate in ongoing education and professional development to stay current with best practices
Requirements
  • Current Ohio RN License
  • Minimum of 1 year of recent home health nursing experience
  • OASIS/Medicare Certified Visits experience
  • Electronic Charting experience
  • American Heart Association BLS
  • Valid U.S. Driver's License
