We are seeking a highly skilled and compassionate Social Worker to join our team at US Renal Care. As a Dialysis Social Worker, you will play a vital role in supporting patients living with kidney disease and their families.
Key Responsibilities- Conduct patient assessments and develop care plans to ensure optimal social functioning and psychological adjustment to dialysis treatment and rehabilitation.
- Advocate for patients' needs and coordinate communities of support, identifying and counseling psychosocial issues and providing patient and family education.
- Collaborate with interdisciplinary teams to promote teamwork, educate staff, and provide training on patient psychosocial care.
- Participate in quality improvement initiatives to enhance patient health and minimize missed treatments and hospitalizations.
- Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
- Current licensure (in good standing) in applicable state, unless employed in AZ, PA, or Guam.
- Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
- Basic computer skills, including Microsoft Office (Word, Excel, Outlook).
- Previous experience in providing social services to dialysis patients.
