About the Role

RHA Health Services is seeking a highly skilled and compassionate IDD Qualified Professional to join our team. As a key member of our interdisciplinary service team, you will play a vital role in guiding the development of person-centered plans and programs for individuals with intellectual and developmental disabilities.

Key Responsibilities
  • Coordinate IDT meetings and develop individualized service plans (ISPs) that address the unique needs and goals of each individual.
  • Assist individuals in identifying and communicating their requests and needs for services and supports through direct interviews and assessments.
  • Supervise, integrate, and coordinate person-centered plans, monitoring progress towards personal, clinical, and functional outcomes.
  • Initiate periodic reviews, investigations, modifications, and adjustments to ensure that individuals' needs and goals are being met.
  • Review assessments and develop person-centered plans that promote independence, health, and well-being.
  • Oversee residential activities, including home appearance, leisure materials, cleanliness, and community integration, to ensure a safe and healthy environment.
  • Visit vocational centers, residential homes, and other service areas to assess the implementation of person-centered plans.
  • Communicate effectively with colleagues, administrators, and individuals' families and guardians to ensure seamless service delivery.
  • Monitor activities to ensure quality assurance and state survey improvements.
  • Serve as a liaison between individuals and their families/guardians to gain approval for services provided.
Requirements
  • Bachelor's degree in Social Work, Social Services, Human Services, or Special Education required; Master's degree a plus.
  • Minimum 2 years of experience working with individuals with intellectual and/or developmental disabilities required; 1-year experience if paired with Master's degree.
  • Working knowledge of applicable regulations and laws governing service offerings.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Supervisory/management experience and experience writing person-centered plans preferred.
  • Must be at least 18 years of age.
