Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.

Key Responsibilities
  • Conduct adult and geriatric assessments, medical history reviews, and provide health education to patients in their homes
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes
  • Work in a variety of settings, including home visits, skilled facility visits, and telehealth visits
  • Provide services that meet the needs of at-risk health plan members, including those with complex medical conditions and social determinants of health
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice, including prescription authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
