Per Diem/PRN Home Health Physical Therapist

BAYADA Home Health Care is seeking a per diem/prn Home Health Physical Therapist for a per diem/prn job in Millville, New Jersey.
As a Home Health Physical Therapist with BAYADA, you will play a crucial role in setting the stage for successful patient care, safe transitions, and positive outcomes for patients leaving Inspira Hospitals.
This position is a 13-week commitment (with option to extend) that allows you the flexibility to work a minimum of 8 shifts per month doing home health visits with patients in Cumberland, Gloucester, and Salem Counties.
We provide a premium rate differential plus mileage reimbursement.
As a Home Health Physical Therapist with BAYADA, you will:
- Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns, and appearance of ambulation, strength, and endurance of muscles, balance testing, and safety assessments.
- Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
- Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
- Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
Requirements:
- Active and current license to practice as a professional Physical Therapist (PT) in the state of NJ.
- Previous home health experience required.
- Must commit to a 13-week assignment and a minimum of 8 shifts per month.
- OASIS experience required; previous HCHB experience preferred.
- Ability to travel throughout the region based on our census.
- Ability to deliver patient care with compassion, excellence, and reliability.
Compensation:
Based on experience and type of visit - $200 - $212 per Start of Care visit
